7. Intraperitoneale Chemotherapie in der Behandlung des Ovarial- und Tubenkarzinoms

Zusammenfassung

Die intraperitoneale Chemotherapie stellt eine lokoregionäre Behandlung des Ovarial- und Tubenkarzinoms dar, bei der es auch durch die peritoneale Zytostatikaresorption zu einem systemischen Antitumoreffekt kommt. Dennoch gilt bislang ein Tumorrest ≤ 1 cm nach Primäroperation als Voraussetzung für ihren Einsatz. Drei von vier randomisierten Studien weisen auf einen signifikanten Überlebensvorteil bei Patientinnen mit fortgeschrittener Erkrankung hin, wenn die primäre Chemotherapie intraperitoneal durchgeführt wird. Allerdings hat die letzte große Studie (GOG 252) die Vorteile einer intraperitonealen Verabreichung stark in Frage gestellt. Routinemäßig werden bislang Paclitaxel, Cisplatin und aufgrund der GOG 252 auch Carboplatin empfohlen, die über ein implantiertes Port-a-Cath-System verabreicht werden. Die intraperitoneale Chemotherapie ist nebenwirkungsreich. Neben katheterassoziierten Nebenwirkungen fallen auch Cisplatin-typische Toxizitäten (wie Emesis und Polyneuropathie) ins Gewicht, die jedoch, durch den Austausch mit Carboplatin, teilweise vermeidbar sind. Die Sonderformen der intraperitonealen Behandlung HIPEC und PIPAC sind sowohl in der Primär- als auch in der Rezidivtherapie klinischen Studien strikt vorbehalten.

Umfrage: Topika mit Dexpanthenol bei Radiodermatitis empfohlen

In der topischen Prävention der akuten Radiodermatitis werden Zubereitungen mit Dexpanthenol oder Harnstoff von deutschsprachigen Fachkreisen in der Radioonkologie am häufigsten empfohlen und als am wirksamsten bewertet. Handekzem: Adhärenz bei topischer Therapie nicht zufriedenstellend

Einer klinischen Studie zufolge wendet nur etwa die Hälfte der Ekzem-Patient:innen ihre topische Therapie mit Kortikosteroiden wie verordnet an.
