Zusammenfassung

Medikamentenfehler stellen eine bedeutsame Bedrohung für Patienten aller Altersgruppen dar. Weil bei Kindern altersgruppenspezifische Besonderheiten bestehen, eine individuelle Berechnung der zu gebenden Dosis erforderlich ist und oft keine Vertrautheit mit der zu verabreichenden Dosis entstehen kann, treten Fehler häufiger auf als bei Erwachsenen. Besonders in Notfallsituationen kommt es neben der psychischen Belastung zur Notwendigkeit, in großer zeitlicher Dichte Medikamente zu verabreichen, die potenziell erhebliche Gefahren bergen. Schon z. B. das falsche Setzen eines Kommas bei der Berechnung einer Adrenalindosis kann lebensbedrohlich sein. Besonders präklinisch werden Kinder oft durch nicht speziell pädiatrisch geschulte Notfallteams versorgt. Der vorliegende Beitrag analysiert den Verordnungs- und Verabreichungsprozess von Medikamenten, zeigt mögliche Fehlerquellen auf und nennt Maßnahmen, die zur Verbesserung der Medikamentensicherheit bei Kindernotfällen beitragen können.
