Zusammenfassung

Lungenkarzinome sind die führende Ursache tumorbedingter Mortalität weltweit. Neueren wissenschaftlichen Studien zufolge sind histologisch ähnliche Tumoren genetisch heterogener als bisher vermutet. Hierdurch bieten sich Ansatzpunkte für „personalisierte“ Behandlungsstrategien, welche die stagnierenden Ergebnisse der ungezielt eingesetzten Chemotherapie verbessern können. Mit der Definition molekularer Zielstrukturen und entsprechender Biomarker wurde für Adenokarzinome der Lunge ein Paradigmenwechsel eingeleitet, Ähnliches ist für Plattenepithelkarzinome bereits erkennbar. Für einige klassische Zytostatika, die zumeist weiterhin die Basis der medikamentösen Therapie des Lungenkarzinoms darstellen, wurden histologische und molekularpathologische Prädiktoren des therapeutischen Nutzens erarbeitet. Aufgrund des immensen Fortschritts der tumorbiologischen, pharmakologischen und diagnostischen Entwicklung ist zu erwarten, dass künftig für die Mehrzahl der Patienten mit Lungenkarzinomen an deren „Tumorprofile“ angepasste medikamentöse Behandlungen angeboten werden. Diese neue Komplexität bedingt höhere Anforderungen an Ausbildung und Expertise von Thoraxonkologen, denen in der Regel nur im interdisziplinären Team spezialisierter Lungenkrebszentren entsprochen werden kann.

Nicht bei allen Prostatakarzinomen der Gleason-Graduierungsgruppe 1 kann man davon ausgehen, dass sie benigne sind. In einer Studie aus dem UKE Hamburg hatten auch in dieser Gruppe bestimmte Patienten ein deutlich erhöhtes Risiko für einen ungünstigen Verlauf.

Taugen Risikoscores für Schlaganfall und Blutungen auch bei Krebskranken?

Britische Registerdaten sprechen dafür, dass sich die Blutungsrisikoprädiktion bei Personen mit Vorhofflimmern, die zugleich an Krebs leiden, nicht einfach aus Kohorten ohne Krebs übertragen lässt.
