Akupunktur und medizinisch-radiologische Bildgebung – Teil 1

Zusammenfassung

Akupunktur als therapeutische Maßnahme und die Radiologie als ein im Schwerpunkt diagnostisches Fachgebiet zeigen trotz dieser Dichotomie in Klinik, Wissenschaft und Lehre mehrere Kontaktebenen. Diese lassen sich differenzieren in die Bereiche bilddiagnostische Abklärung akupunkturassoziierter Komplikationen und Therapiewirkung (funktionell und strukturell) sowie die regionalradiologische Kontrolle von Nadelpositionierung und die Untersuchung von Akupunkturpunkten. Diese Themenbereiche werden in diesem Review in ihren Schwerpunkten differenziert aufgezeigt und diskutiert.
Die gezielte Literaturrecherche enthüllt eine unerwartete Anzahl schwerwiegender Komplikationen bei der Akupunkturbehandlung, von denen vor allem Pneumothorax und die intrakorporale Nadelmigration mit vielfältigen Migrationswegen und teils vital gefährlichen Residualpositionierungen, welche oft einer aufwendigen, bildgestützten chirurgischen Resektion bedürfen, in Betracht kommen.
In Forschung und Lehre könnte die Sonographie ein nützliches Instrument sein, um Variationen bezüglich erforderlicher Nadeleinstecktiefe und -eintrittswinkel und der anatomischen Strukturen zwischen Individuen zu untersuchen. Retrospektive Computertomografie-(CT-)Studien können für die Akupunkturforschung ein wichtiges Instrument sein, spezifische Fragen zur anatomischen Lagebeziehung von kutanem Nadeleindringepunkt und Zielstruktur bzw. sensiblen Organstrukturen zu klären.
Moderne biochemische Bildgebungstechniken wie Positronen-Emissions-Tomographie (PET), Einzelphotonenemissions-CT (SPECT), funktionelle Magnetresonanztomographie (fMRI) können neue Perspektiven für die Validierung der Akupunkturspezifität, die Optimierung der Akupunkturanalgesie und die Aufdeckung des zentralen Mechanismus der Akupunkturanalgesie bieten und auch in nichtneuralen Organregionen funktionelle Wirkungen der Akupunktur bildgestützt aufzeigen.
