Aktuelle Trends der invasiven und nichtinvasiven Pränataldiagnostik

Zusammenfassung

Das Screening im ersten Trimester, bestehend aus qualifizierter Beratung und differenzierter Ultraschalldiagnostik, stellt heutzutage die Grundlage der Entscheidung über den Umfang möglicher weiterer Untersuchungen in der Schwangerschaft dar. Etwa die Hälfte relevanter fetaler Fehlbildungen ist zu diesem Zeitpunkt bereits diagnostizierbar. Eine erweiterte Nackentransparenz (NT) weist nicht nur auf ein erhöhtes Risiko für Aneuploidien, sondern auch auf andere, seltenere, genetische Erkrankungen oder strukturelle Fehlbildungen hin. Das Spektrum der pränatalen Diagnostik im ersten Trimester wurde durch die frühe Abschätzung des individuellen PE(Präeklampsie)-Risikos um einen wesentlichen Punkt ergänzt. Die Beratung der Schwangeren gestaltet sich somit zunehmend komplex und muss den aktuellen Möglichkeiten des Screenings, bis hin zur diagnostischen Abklärung mit der Möglichkeit weiterführender genetischer Analysen, Rechnung tragen, vor allem aber den individuellen Wünschen und Bedenken der Schwangeren.
