Zusammenfassung

Zu unterscheiden ist die akute unkomplizierte von der akuten komplizierten Pyelonephritis. Letztere tritt v. a. bei älteren Patienten u. a. mit Anomalien der Harnwege, Reflux, bei Nierensteinen, operativen Eingriffen einschließlich Transplantation oder Diabetes mellitus auf. Die Definition der chronischen Pyelonephritis ist nicht eindeutig - meist gilt eine chronisch atrophische Pyelonephritis mit Gewebeveränderungen (z. B. Narben) als chronisch. Auch Nierenkarbunkel oder Nierenabszesse treten häufiger bei Frauen als bei Männern auf, sie werden begünstigt durch eine komplizierte Harnwegsinfektion bei Vorliegen von Harnwegsanomalien, nach urologischen Operationen, bei Immunschwäche u. a. Beschrieben werden zudem die renale Papillennekrose, die infizierte Harnstauungsniere, xanthogranulomatöse und emphysematöse Pyelonepritis, die Malakoplakie der Niere sowie Mykosen der Niere.
