Reaktive Arthritis – ein Update

Zusammenfassung

Die reaktive Arthritis, eine Form der seronegativen Spondylarthritis, tritt nach einer urogenitalen oder gastrointestinalen Infektion auf und ist durch eine asymmetrische Oligoarthritis gekennzeichnet. Extraartikuläre Manifestationen wie Fieber, Konjunktivitis oder Hautaffektionen können vorkommen. Die Diagnose – insbesondere die Abgrenzung zur septischen Infektion des Bewegungsapparates – kann schwierig sein; für die Behandlung ist die Unterscheidung elementar. Die Ätiopathogenese der reaktiven Arthritis ist noch nicht vollständig verstanden. Bakterielle Fragmente – sogar lebensfähige Chlamydien – wurden innerhalb des Gelenks isoliert. Die meisten randomisierten Studien haben jedoch keine Wirksamkeit einer Antibiotikatherapie nachgewiesen. Die Krankheit kann chronisch und destruierend verlaufen, sodass eine Langzeittherapie nötig wird. Neben krankheitsmodifizierenden entzündungshemmenden Medikamenten scheinen Biologika in diesen Fällen wirksam zu sein.
