Weaning von invasiver Beatmung

Herausforderungen im klinischen Alltag

Zusammenfassung

Die moderne Intensivmedizin bewegt sich im Spannungsfeld einer leistungsfähigen, jedoch auch zunehmend technisierten Intensivtherapie mit zahlreichen therapeutischen Optionen und zugleich einer älter werdenden Gesellschaft mit steigender Morbidität. Dies zeigt sich u. a. in der steigenden Zahl beatmeter Patienten auf den Intensivstationen und zugleich in einem zunehmenden Patientenanteil, bei dem die Beatmung nicht ohne Weiteres beendet werden kann. Der Beatmungsentwöhnung (Weaning), die einen Anteil von mehr als 50 % der gesamten Beatmungsdauer ausmachen kann, kommt eine zentrale Rolle in diesem Prozess zu. In diesem Leitthemenbeitrag wird die Notwendigkeit eines strategisch klugen und ganzheitlichen Handelns dargestellt, um die Folgen invasiver, maschineller Beatmung für die Patienten gering zu halten. Es soll der Versuch unternommen werden, einzelne Aspekte der Beatmungsentwöhnung mit hoher Relevanz für die klinische Praxis näher zu beleuchten. Insbesondere für das prolongierte Weaning sind weitaus mehr Konzepte erforderlich, als die Beatmung schlicht zu beenden.
