13. Rezidivbelastungsinkontinenz

Zusammenfassung

Verschiedene Komplikationen der Inkontinenzchirurgie wie Obstruktion, De-novo-Urge-Inkontinenz, Extrusion von Schlingen oder Erosion des Bandes in die Blase oder Harnröhre können auftreten. Ursachen des Versagens der Primärtherapie können technische Probleme bei der Erstoperation, inkorrekte Wahl der Prozedur, persistierend hoher intraabdominaler Druck oder „schlechte“ Gewebeverhältnisse bei der Erstoperation sein. Eine genaue Ursachenforschung für das Versagen der Primäroperation ist für den Erfolg einer Rezidivoperation entscheidend. Die exakte Aufklärung der Patientin beim Primäreingriff bei postoperativ weiterbestehenden Risikofaktoren wie chronischer Raucherhusten oder Adipositas sind aus forensischen Gründen wichtig. Meist ist die Anwendung der gleichen Operationstechnik, die bei der Primäroperation verwendet wurde, nicht sinnvoll. Am einfachsten und besten geeignet bei Rezidivstressinkontinenz ist die Faszienzügelplastik aus allogenem Material.
