16. Senkung des vorderen Vaginalkompartiments

Zusammenfassung

Die pelvinen Organe werden durch muskuläre und bindegewebige Strukturen im knöchernen Becken gehalten und formen eine stabile Einheit. Ist eine der Strukturen defekt, so kann dies zur Senkung der Vaginalwand und der Organe durch den Hiatus urogenitale führen. Bei einem zentralen Defekt resultiert eine Pulsionszystozele. Ziel der operativen Therapie ist der Verschluss des zentralen Defektes der Vaginalwand unter dem Blasenboden zur Versenkung der Pulsionszystozele. Somit wird sowohl die normale vaginale Anatomie als auch eine normale Blasenfunktion wiederhergestellt bzw. erhalten. Die Rekonstruktion einer Traktionszystozele hat die Wiederherstellung des lateralen Vaginalsulkus durch Behebung des paravaginalen Defektes zum Ziel. Die Indikation zur operativen Versorgung der Traktionszystozele mit ein- oder beidseitigem Abriss der lateralen vaginalen Aufhängung ist von der Beschwerdesymptomatik der Patientin abhängig.
