Axiale Spondyloarthritis (einschließlich ankylosierender Spondylitis)

Zusammenfassung

Der Begriff Spondyloarthritis (SpA) umfasst eine Gruppe von rheumatischen Erkrankungen, die neben einer Entzündung des Achsenskelettes (Sakroiliitis, Spondylitis) auch durch eine Entzündung peripherer Gelenke (Arthritis) und Sehnenansätze (Enthesitis) und durch eine Assoziation mit dem MHC-Klasse-I-Antigen HLA B27 charakterisiert ist. Die SpA wird heute je nach im Vordergrund stehender klinischer Symptomatik in eine prädominant axiale und in eine prädominant periphere Form eingeteilt. Bei allen Patienten mit SpA können eine Beteiligung weiterer Organe wie den Augen (anteriore Uveitis), der Haut (Psoriasis) und des Darmes (Morbus Crohn oder Colitis ulcerosa) vorliegen. Die Therapie der axSpA basiert auf regelmäßig durchzuführenden Bewegungsübungen und Physiotherapien und einer ggf. kontinuierlich durchzuführenden Therapie mit nichtsteroidalen Antirheumatika (NSAR). Bei Patienten, die auf diese Standardtherapie nicht angesprochen haben, sollte die Einleitung einer Therapie mit TNF-α-(TNF)-Blockern in Erwägung gezogen werden.
