Colchizin in der pädiatrischen Rheumatologie

Pädiatrische Rheumatologie
Colchizin ist als pflanzlicher Wirkstoff seit Jahrhunderten im Einsatz gegen den akuten Gichtanfall. Seit den 1970er-Jahren zeigte sich seine sehr gute Wirksamkeit beim familiären Mittelmeerfieber (FMF) und ist für diese Indikation der Goldstandard. Neuere Indikationen sind die rekurrierende Perikarditis sowie weitere kardiovaskuläre Erkrankungen. Colchizin wirkt stark entzündungshemmend, indem es die Zellen der angeborenen Immunität (Makrophagen, Neutrophile, Monozyten) insbesondere durch Zerstörung des Mikrotubuliapparats in vielfältigen Funktionen hemmt. Der effektiven und nebenwirkungsarmen Anwendung steht eine geringe therapeutische Breite gegenüber. Ein konsequentes Therapiemonitoring und die Vermeidung kritischer Komedikationen sind zur Verhinderung von Intoxikationen unerlässlich.
