Zusammenfassung

Regelmäßige körperliche Aktivität bei Typ-1-Diabetes ist mit positiven Effekten auf das Herz-Kreislauf-System und die Gesamtmortalität assoziiert und stellt daher einen essenziellen Teil des modernen Diabetesmanagements dar. Der zugrunde liegende Insulinmangel und die deswegen notwendige Zufuhr von exogenem Insulin führen zu einer Disbalance der physiologischen Glukosehomöostase, die sich v. a. im Rahmen von Sport in akuten Problemen wie Hypoglykämien niederschlägt. Spezifische Anpassungen der Insulintherapie sowohl im Rahmen sportlicher Betätigung als auch in der Vorbereitungs- und Erholungsphase sind in Abhängigkeit der Art, Dauer und Intensität der körperlichen Aktivität zu planen und erfordern individuelle und durch Evidenz belegte Therapieempfehlungen. Der Einfluss körperlicher Aktivität auf (patho)physiologische Mechanismen der Blutzuckerkontrolle bei Typ-1-Diabetes und der aktuelle Status spezifischer Empfehlungen zur Anpassung der Insulintherapie bei Sport werden vorgestellt.
