CKD-G 3–5: Antikoagulation bei Vorhofflimmern und Thromboembolien

Zusammenfassung

Die Entscheidung über eine Antikoagulation bei Patienten mit chronischer Nierenerkrankung (CKD) und Vorhofflimmern ist erschwert, da diese Patienten ein klar erhöhtes Risiko für Blutungen und thrombotische Ereignisse aufweisen. Nur bei CKD-Grad 3 a/b besteht evidenzbasierter Konsensus, dass bei einem CHADS₂-VASc-Score von 2 oder mehr (Männer) bzw. von 3 oder mehr (Frauen) eine Antikoagulation mittels direkter oraler Antikoagulanzien (DOAK) indiziert ist; nur Patienten mit mechanischem Klappenersatz oder mittel- bis schwergradiger Mitralklappenstenose benötigen Vitamin-K-Antagonisten (VKA). Ein ähnliches Prozedere wird auch für Patienten mit CKD-Grad 4 vorgeschlagen. Bei Dialysepatienten ist die Indikation zur Antikoagulation höchst umstritten, da der Nutzen (Reduktion von ischämischen Ereignissen) unklar und das Risiko (Blutungen) deutlich höher ist als bei nicht dialysepflichtigen Patienten. Wenn die Entscheidung zur Antikoagulation getroffen ist, sind auch hier dosisangepasste DOAK den VKA meist überlegen.
