Zusammenfassung

Das selten beschriebene benigne subkutane Emphysem (BSKE) kommt in verschiedensten Körperregionen vor. Als Ursache im Bereich der Extremitäten wird häufig ein gelenknahes „air entrapment“ genannt. Unfälle mit pneumatischen Geräten wurden ebenfalls als Ursache berichtet. Hinsichtlich der Therapie müssen lebensbedrohliche Emphyseme infektiöser Genese ausgeschlossen werden; dies ist insbesondere in der Frühphase nicht immer sicher möglich. Während der Erstbehandlung in der Notaufnahme steht neben dem klinischen Bild der Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) Score zur Verfügung. Bei Zweifeln an der Gutartigkeit des Emphysems ist frühzeitig ein mit Weichteilinfektionen vertrauter Chirurgen zu konsultieren. Der frühzeitige sichere Ausschluss einer potenziell lebensbedrohlichen gasbildenden Weichteilinfektion wie der nekrotisierenden Fasziitis gelingt manchmal nur über die chirurgische Exploration, die dem Patienten bei adäquater Diagnose bereits in der Notaufnahme erspart bleibt.
