Venöse Thromboembolien, Thrombophilie und Thromboseprophylaxe in der Geburtshilfe

Zusammenfassung

Obwohl Venenthrombosen und Lungenarterienembolien mit einer Inzidenz von etwa 1,2 pro 1000 Entbindungen seltene Ereignisse sind, sind tödliche Lungenarterienembolien die häufigste Ursache für Müttersterblichkeit in Industrieländern. In mindestens der Hälfte aller Fälle sind vor Eintritt einer Thromboembolie Risikofaktoren erkennbar, sodass diese Ereignisse vermeidbar wären. Schwangerschaftsassoziierte Thromboserisikofaktoren sollten bereits präkonzeptionell strukturiert erfasst und quantifiziert werden; bei hohem Risiko sollte antenatal eine prophylaktische Antikoagulation erfolgen. Physiologische Veränderungen während der Schwangerschaft erschweren das Erkennen von Thromboemboliesymptomen und die sichere Diagnostik. Deshalb sollten etablierte Empfehlungen befolgt werden.
