Resektionsarthroplastik und Arthrodese in der Prothesenrevision

Zusammenfassung

Sowohl die Schulterarthrodese als auch die definitive Resektionsarthroplastik können als Reserveverfahren zur Behandlung von schweren Komplikationen nach Schulterprothesenimplantation zum Einsatz kommen. Eine adäquate Indikationsstellung und strenge Patientenselektion sind für beide Verfahren entscheidend. Die Indikation für die Arthrodese besteht im Falle eines Prothesenversagens beim jungen Patienten oder bei chronischer neurologisch-muskulärer Schädigung. Im Rahmen der operativen Versorgung stehen die Auffüllung des knöchernen Defekts durch autologes oder allogenes Knochenmaterial sowie die osteosynthetische Primärstabilität im Vordergrund. Die Nachsorge der Arthrodese umfasst eine 12-wöchige Ruhigstellung und Aufbelastung nach knöcherner Konsolidierung. Die definitive Resektionsarthroplastik ist multimorbiden Patienten i. d. R. nach Infektgenese vorbehalten. Dabei stellen ein ausgiebiges Débridement und die anschließende adäquate systemische Antibiotikatherapie die notwendigen Maßnahmen dar.
