Implantate beim älteren Patienten

Zusammenfassung

Ältere Patientengruppen unterscheiden sich fundamental voneinander und stellen die am schnellsten wachsenden Patientengruppen dar. Menschen im dritten Lebensalter kommen heute aus der sog. Babyboomer-Generation und haben oftmals hohe Ansprüche an ihre zahnmedizinische Versorgung. Für sie steht das gesamte Spektrum der zahnärztlichen Implantologie zur Verfügung, allerdings müssen chronische Erkrankungen, deren Medikation und die manuelle Geschicklichkeit beachtet werden. Da dentale Implantate meist deutlich länger als 20 Jahre im Mund verbleiben, muss prospektiv die Abnahme der biologischen, sozialen und psychischen Funktionen bei der Planung berücksichtigt werden. Menschen im vierten Lebensalter gehören häufig der Elterngeneration der Babyboomer an. Ihr Leben ist oft von Abhängigkeit bei den Aktivitäten des täglichen Lebens (ADL), Multimorbidität, Polypharmazie und Institutionalisierung bestimmt. Obwohl es fast keine wissenschaftliche Evidenz zur Implantattherapie bei diesen hochaltrigen Menschen gibt, sollte ihnen diese nicht von vornherein verwehrt bleiben. Die wenigen vorhandenen Studien zeigen eine sehr hohe Implantatüberlebensrate und es gibt Hinweise auf verringerte akute periimplantäre Entzündungsreaktionen im Vergleich zu jüngeren Implantatpatienten. Allerdings müssen die Handhabung, Nachsorge und tägliche Pflege gesichert werden, bevor mit einer solchen Therapie begonnen wird.
