Up to 18 % of ankle sprains are associated with deltoid ligament and syndesmotic injury [
8]. However, ankle diastasis without fibular fracture is rarely reported in literature and mostly in athletes [

4]. This injury usually occurs with forced pronation-abduction or pronation and eversion of the foot, leading to external rotation or abduction in the ankle.
