Depression und Epilepsie

Zwei Krankheitsbilder mit gemeinsamen Ursachen?

Zusammenfassung

Hintergrund

Epilepsien und depressive Störungen zeigen eine hohe Komorbiditätsrate. Daher werden neurobiologische Gemeinsamkeiten sowie ein bidirektionaler Zusammenhang hinsichtlich der Ätiopathogenese angenommen.

Ziel der Arbeit

Das Ziel der Arbeit ist die Darstellung der neurobiologischen Gemeinsamkeiten beider Erkrankungen, die Charakterisierung ihres bidirektionalen ätiologischen Zusammenhangs sowie die Diskussion der therapeutischen Konsequenzen.

Material und Methoden

Es wurde eine Literaturrecherche durchgeführt und Arbeiten über die Interaktion von Depressionen und Epilepsien ausgewertet.

Ergebnisse und Diskussion

Epilepsien und depressive Störungen zeigen einige gemeinsame neurobiologische Merkmale. Bei Patienten mit Epilepsie sollte eine Depression früh und verlässlich erkannt werden, da die erfolgreiche Behandlung Einfluss auf die Prognose, die Lebensqualität und das erhöhte Suizidrisiko der Erkrankten hat. Eine antidepressive Therapie mit noradrenergen spezifisch serotonergen Antidepressiva (NaSSA) oder selektiven Serotonin-Reuptake-Inhibitoren (SSRI) hat im therapeutischen Dosisbereich kein klinisch relevantes epileptogenes Potenzial. Sie steigert jedoch häufig die Lebensqualität und könnte zudem antikonvulsive Effekte bei Patienten mit Epilepsie zu haben. Clomipramin, Bupropion und Maprotilin sollten bei Patienten mit bekannter Epilepsie aufgrund ihrer die Anfallsschwelle senkenden Eigenschaft vermieden werden.
