Was ist anders – wie lässt sich eingreifen?

Zusammenfassung

Herzinsuffizienz (HI) und Diabetes mellitus (DM) sind häufige Komorbiditäten, und Letzterer erhöht das Risiko für die Entstehung Ersterer durch eine Induktion von Mikro- und Makroangiopathie, aber auch Veränderungen des kardialen Metabolismus. Andererseits haben Patienten mit HI ein deutlich erhöhtes Risiko für die Entwicklung von Insulinresistenz und im weiteren Verlauf für das Auftreten eines DM aufgrund neuroendokriner Aktivierung. Trotz dieser Kenntnisse sind die genauen Ursachen für die kardiometabolischen Wechselwirkungen bei HI und DM unvollständig verstanden. Für die Behandlung von DM stehen aktuell verschiedene orale Antidiabetika sowie blutzuckerspiegelsenkende Medikamente zur Auswahl, doch sind deren Auswirkungen auf kardiovaskuläre und insbesondere auf Herzinsuffizienzendpunkte durchaus variabel. Während sich die meisten Substanzen hinsichtlich Herzinsuffizienz neutral verhalten, erhöhen Glitazone das Risiko für eine solche, während Natrium-Glukose-Kotransporter-2-Inhibitoren (SGLT-2-Inhibitoren) zuletzt einen deutlich günstigen Effekt zur Vermeidung einer HI entfalteten. Es wird auf die metabolischen Veränderungen bei DM und HI sowie die Wirkmechanismen der meisten (insbesondere neueren) antidiabetischen Medikamente eingegangen, um hierüber Empfehlungen zur optimalen Therapie von Patienten mit DM und HI zu geben.

SGLT-2-Inhibitoren für alle Patient:innen mit chronischer Herzinsuffizienz empfohlen

Das ESC-Leitlinien-Update 2023 bedeutet einen Paradigmenwechsel in der Behandlung der Herzinsuffizienz (HF), denn nun werden SGLT-2i sowohl für HFrEF, als auch für HFmrEF und HFpEF empfohlen. Somit können jetzt alle Patient:innen mit HF von SGLT-2i als zentralem Bestandteil der Therapie profitieren.

Dapagliflozin als neue Therapieoption zur Behandlung einer Herzinsuffizienz unabhängig von der Ejektionsfraktion

Dapagliflozin ist nun zur Behandlung aller Patient:innen mit chronischer symptomatischer Herzinsuffizienz zugelassen und bietet somit auch neue Hoffnung für die Therapie von jenen mit HFpEF. In der DELIVER-Studie zeigte der SGLT-2-Inhibitor eine signifikante Reduktion von Herzinsuffizienz-Hospitalisierungen und CV-Todesfällen.
