Foot ulcers are a major cause of morbidity and impaired quality of life in patients with diabetes mellitus, and they represent a major work load and cost for healthcare systems. This article develops a stepped-care approach (fig. 1), and gives practical guidelines for ulcer preparation, dressing and protection, which we hope will be useful.
