Diabetische Neuropathie

So erkennen und behandeln Sie die verschiedenen Formen

Zusammenfassung

Die diabetische Polyneuropathie gehört zu den häufigsten Komplikationen des Diabetes mellitus und umgekehrt ist der Diabetes mellitus die häufigste Ursache einer Polyneuropathie in Europa und den USA. Die häufigste Form ist die distal-symmetrische Polyneuropathie, jedoch gibt es noch weitere Formen der diabetischen Neuropathie. Die Diagnostik der diabetischen Polyneuropathie kann insbesondere dann eine Herausforderung darstellen, wenn seltenere Formen vorliegen oder eine atypische Manifestation die Abgrenzung gegenüber alternativen Neuropathieursachen erschwert. Eine kausale Therapieoption ist außer der Optimierung der Blutzuckereinstellung und von Lifestyle-Faktoren nicht verfügbar, von hoher klinischer Relevanz ist jedoch die Behandlung neuropathischer Schmerzen, die ca. die Hälfte der Patienten mit diabetischer Polyneuropathie betreffen. Auch die Symptome der autonomen diabetischen Neuropathie, die verschiedene Organsysteme betreffen kann, erfordert häufig eine symptomatische Therapie.
