Diabetische Polyneuropathie

Zusammenfassung

Etwa jeder 3. Mensch mit Diabetes ist von der distal-symmetrischen sensomotorischen Polyneuropathie (DSPN) betroffen, die unter Ausbildung teils quälender neuropathischer Schmerzen, andererseits schmerzloser Fußulzera mit erheblicher Einschränkung der Lebensqualität einhergeht. Bis zu 1/4 der Diabetespatienten leiden an einer schmerzhaften DSPN, bis zur Hälfte der Betroffenen können asymptomatisch sein. Leider ist die DSPN nach wie vor unterdiagnostiziert. Typische neuropathische Symptome sind Schmerzen, Parästhesien und Taubheitsgefühl v. a. in Füßen und Unterschenkeln. Die Therapie beinhaltet: 1. Lebensstiländerung, kausale Therapie mit nahezu Normoglykämie als Ziel, multifaktorielle kardiovaskuläre Risikointervention, 2. pathogenetisch begründete und 3. symptomatische Behandlung neuropathischer Schmerzen. Die multimodale Schmerztherapie sollte neben der Schmerzlinderung auch auf eine Verbesserung der Schlafqualität, der Mobilität und der allgemeinen Lebensqualität abzielen.
