Myokardiale Perfusionsdiagnostik der koronaren Herzerkrankung

Zusammenfassung

Die technologischen Fortschritte der letzten Jahrzehnte haben eine Vielzahl nichtinvasiver bildgebender Verfahren zur Diagnose der koronaren Herzkrankheit (KHK) hervorgebracht. Die hohe diagnostische Genauigkeit sowie die prognostische Aussagekraft von Perfusionsmessungen mittels Magnetresonanztomographie (MRT), „single photon emission computed tomography“ (SPECT) und Positronenemissionstomographie (PET) sind in vielen Einzelcenter- und auch Multicenterstudien belegt. Deshalb sind diese Verfahren in den europäischen Leitlinien für Patienten mit intermediärer KHK-Vortestwahrscheinlichkeit empfohlen worden. Dagegen sollte die Perfusionsmessung mittels Computertomographie (CT) aufgrund methodischer Aspekte sowie wegen noch mangelnder Erfahrungswerte nicht in der klinischen Routinediagnostik eingesetzt werden. Die Ischämiediagnostik mittels Hybridverfahren wird heutzutage nur begrenzt in speziellen Zentren zur KHK-Diagnostik verwendet.
