Zusammenfassung

Hintergrund

Die Adressierung von Zelloberflächenrezeptoren auf Tumorzellen ermöglicht hochspezifische Diagnostik und Therapie in der Nuklearmedizin. Radiopharmaka reichern sich gezielt im Tumorgewebe an und können es durch Bestrahlung von innen zerstören. Gesundes Gewebe wird hier maximal geschont, da sich hohe Kontraste zwischen Tumor und gesundem Gewebe ergeben.

Ziel der Arbeit

In diesem Beitrag wird eine Auswahl an neuen und etablierten Entwicklungen im Bereich der nuklearmedizinischen Theranostik vorgestellt und der aktuelle klinische Stellenwert erläutert.

Material und Methode

Relevante Artikel wurden über eine Recherche in der PubMed-Datenbank identifiziert. Informationen zu klinischen Studien stammen aus dem Studienregister Clinicaltrials.gov, Arzneimittelzulassungen aus den öffentlichen Schreiben der European Medicines Agency (EMA) und der US Food and Drug Administration (FDA).

Ergebnisse

In der Nuklearmedizin gibt es derzeit zahlreiche neue Forschungsansätze, und das Konzept der Theranostik etabliert sich immer mehr im klinischen Alltag. Die NETTER-1-Studie (NCT01578239) führte zur Zulassung eines Radiopharmakons zur Behandlung von neuroendokrinen Tumoren, während eine erfolgreiche Phase-III-Studie (VISION, NCT03511664) den Stellenwert des Theranostikums PSMA-617 für die Diagnostik und Therapie des Prostatakarzinoms unterstreicht.

Schlussfolgerungen

Die Radiotheranostik ist in den letzten Jahren in den Fokus der onkologischen Therapie gerückt und unterstützt mit innovativen Therapieverfahren die Onkologie der Zukunft. Das theranostische Konzept bietet viele Vorteile für den Patienten und den behandelnden Arzt; es steht mit einer individuellen Dosimetrie und Diagnostik sowie therapiebegleitenden Kontrollen für eine moderne personalisierte Krebsmedizin.

Bei Immuntherapien das erhöhte Thromboserisiko beachten

Unter modernen Systemtherapien versechsfacht sich das VTE-Risiko. Warum diese Daten relevant für die Behandlung krebsassoziierter Thrombosen sind, erläutert Prof. F. Langer im Interview. So kann es durch Immuntherapien zu inflammatorischen Syndromen z.B. im GI-Trakt kommen. Nebenwirkungen wie Durchfall oder Mukositis haben dann Einfluss auf die Wirksamkeit oraler Antikoagulantien. Aber auch in punkto Blutungsrisiko ist Vorsicht geboten. Wann hier bevorzugt NMH eingesetzt werden sollten, erläutert Prof. Langer im Interview.

CAT-Management ist ganz einfach – oder doch nicht?

Krebsassoziierte venöse Thromboembolien (CAT) haben in den vergangenen Jahren stetig zugenommen. Was hat der Anstieg mit modernen Antitumortherapien zu tun?
