Zusammenfassung

Die akute Pankreatitis ist eine potenziell lebensbedrohliche Erkrankung, insbesondere bei schweren Verläufen. Sie wird morphologisch in die interstitielle ödematöse oder nekrotisierende Pankreatitis eingeteilt. Gemäß der revidierten Atlanta-Klassifikation werden aufgrund der lokalen und systemischen Komplikationen sowie der begleitenden Organversagen milde, moderate und schwere klinische Verläufe differenziert. In der Initialphase der Therapie steht die Basistherapie mit (aggressiver) Volumensubstitution, frühzeitiger enteraler Ernährung und differenzierter Schmerztherapie im Vordergrund. Charakteristisch sind im Verlauf der schweren akuten Pankreatitis abdominelle Nekrosen, die eine individualisierte und interdisziplinäre Behandlung mit Antibiotika, Drainagenanlage und definitiver Nekrosektomie erfordern. Die Nekrosektomie sollte als „Step-up-Verfahren“ geplant werden, wobei interventionell-radiologische, endoskopische und minimal-invasive chirurgische Maßnahmen angewendet werden.
