5. Diagnostik und Therapie bei diabetisch-neuropathischer Osteoarthropathie

Zusammenfassung

Die diabetische neuropathische Osteoarthropathie (DNOAP), im klinischen Alltag meist »Charcot-Fuß« genannt, stellt als nichtinfektiöse Zerstörung von Knochen und Gelenken im Zusammenhang mit der Neuropathie eine Sonderform des diabetischen Fuß-Syndroms dar. Die DNOAP ist eine chronische und progressiv verlaufende Erkrankung der Knochen und Gelenke, charakterisiert durch entweder schmerzhafte oder schmerzlose Destruktion, hautsächlich im Bereich des Fußes lokalisierter Knochen- und Gelenkstrukturen. Häufig wird diese Spätkomplikation des Diabetes mellitus zumindest in den Anfangsphasen verkannt und führt unsachgemäß behandelt zu einer progredienten Zerstörung des Fußskeletts und zur Invalidität der betroffenen Patienten. Im akuten Stadium ist eine komplette Druckentlastung des Fußes, im chronischen Stadium eine entsprechende Maßschuhversorgung oder ggf. eine operative Korrektur zur Vermeidung von Fußulzerationen anzustreben.
