Diagnostik und Therapie von Chlamydia-trachomatis-Infektionen

Zusammenfassung

Nukleinsäureamplifikationstests gelten heute als Methode der Wahl zum Nachweis von Chlamydia trachomatis. Sie besitzen die höchste Sensitivität und eine hohe Spezifität, die nahezu der der Kultur entspricht. Bei Männern mit urogenitalen Infektionen stellt Erststrahlurin das bevorzugte Untersuchungsmaterial dar, bei Frauen sind vaginale und zervikale Abstrichproben mindestens gleichwertig. Chlamydien-Schnelltests („point-of-care-tests“) können in kurzer Zeit und ohne großen apparativen Aufwand durchgeführt werden, sind aber aufgrund mangelnder Genauigkeit für die Diagnostik ungeeignet. Serologische Tests sind für die akute Infektion ebenfalls nicht geeignet, können aber im Rahmen der Diagnostik chronisch invasiver Infektionen nützlich sein, da die Erreger dann nicht mehr zuverlässig in Urin oder Abstrichproben nachweisbar sind. Entsprechend verschiedenen Richtlinien erfolgt die Therapie unkomplizierter Chlamydien-Infektionen primär mit Doxycyclin oder Azithromycin, alternativ auch mit Erythromycin, Ofloxacin oder Levofloxacin. Chronisch persistierende Infektionen erfordern in der Regel eine Behandlung über einen längeren Zeitraum oder durch Kombination verschiedener Antibiotika.
