A 28-year-old woman with a history of chronic migraine developed an ischaemic stroke while receiving daily sumatriptan and intermittent diclofenac for pain control. The woman had been using sumatriptan daily for three years and diclofenac as needed. History included a prior transient neurological episode three years earlier with unremarkable neuroimaging. She presented with acute expressive aphasia and right-sided motor and sensory deficits. Brain MRI revealed two punctate acute ischaemic lesions in the M4 cortical territory of the left middle cerebral artery. Laboratory investigations were largely unremarkable except for subclinical hypothyroidism and borderline dyslipidaemia. The woman received acute stroke management.
