Post-mortem-Nachweis diffuser Axonschäden durch Immunhistochemie

Zusammenfassung

Einleitung

Schädel-Hirn-Traumata, welche in Deutschland hauptsächlich durch Verkehrsunfälle verursacht werden, umfassen als eine mögliche Verletzung den diffusen Axonschaden (DAI). Hohe Kopfbeschleunigungen können zur Dehnung von Axonen in mehreren Hirnarealen führen, wobei es dadurch innerhalb der Axone zu einer Akkumulation von Transportstoffen (z. B. β‑APP) kommen kann. Ziel dieser Arbeit war es, DAI post mortem durch Antikörperfärbung zu diagnostizieren.

Material/Methode

Es wurde Gehirngewebe von 26 Verkehrsunfallopfern sowie 2 Sturzopfern histologisch untersucht, wobei prospektiv und retrospektiv Proben aus mehreren Hirnarealen entnommen wurden. Die Immunhistochemie wurde mit Antikörpern gegen β‑APP durchgeführt, wobei weiterführend eine Differenzierung zwischen hypoxisch und traumatisch bedingten Axonverletzungen durch Unterscheidung zweier Färbemuster vorgenommen wurde.

Ergebnisse

Es wurden in 12 Verkehrsunfallopfern und einem Sturzopfer DAI diagnostiziert, wobei anhand der Verteilung der gefärbten Axone traumatisch und hypoxisch bedingte Verletzungen unterschieden werden konnten. Der traumatische Axonschaden wurde vorwiegend im Balken, in der Brücke und der inneren Kapsel nachgewiesen. Die Überlebenszeit der Opfer betrug 5 h bis 34,5 Tage. Alle 13 Fälle hatten schwere Kopfverletzungen erlitten, wobei 10 der Opfer an einem Schädel-Hirn-Trauma verstarben.

Diskussion/Zusammenfassung

Eine Immunhistochemie mit Antikörpern gegen β‑APP bestätigte sich als zuverlässige Methode, um DAI post mortem zu diagnostizieren und um traumatisch bedingte Axonschäden von hypoxischen abgrenzen zu können. Der Nachweis von DAI in verschiedenen Hirnregionen kann in Zukunft möglicherweise wichtige Hinweise für die Rekonstruktion von Unfallhergängen liefern.
