Schlaganfall unter NOAK-Therapie – was nun?

Zusammenfassung

Das jährliche Risiko, einen akuten ischämischen Schlaganfall bei nicht valvulärem Vorhofflimmern unter einer NOAK-Therapie (Dabigatran, Rivaroxaban, Apixaban, Edoxaban) zu erleiden, beträgt 1–2 %. In 0,23–0,5 % der Fälle kommt es unter NOAK-Therapie zu einer intrakraniellen Blutung.
In diesem Artikel wird das Management eines ischämischen oder hämorrhagischen Schlaganfalls unter NOAK-Therapie dargestellt, insbesondere in Hinblick auf die systemische Thrombolyse, die Stellung der Plasmaspiegelkontrollen und die Antagonisierung. Zudem werden differenzialdiagnostische Überlegungen und praktisch relevante Aspekte hinsichtlich der Wiedereinleitung der Therapie diskutiert.
