Zusammenfassung

Die restaurative Proktokolektomie mit ileopouchanaler Anastomose (IPAA) ist die chirurgische Therapie der Wahl bei Patienten mit therapierefraktärer Colitis ulcerosa (CU) oder Patienten mit CU-assoziierten Präneoplasien und kolorektalen Karzinomen. Sie wird überwiegend laparoskopisch durchgeführt. Die Rekonstruktion mit einem J‑Pouch ist – aufgrund der technischen Einfachheit und der guten Langzeitergebnisse – die am häufigsten angewandte Variante. In der vorliegenden Übersichtsarbeit sollen die möglichen postoperativen Pouchkomplikationen, deren Risikofaktoren, Diagnostik und chirurgisches Management sowie die mittel- und langfristige Lebensqualität nach einer Pouchanlage anhand der aktuellen Literatur differenziert dargestellt werden.
