Adipöser Patient mit Luftnot und Stauungsdermatitis

Ein 67-jähriger Patient stellte sich notfallmäßig wegen einer seit etwa 4 Tagen zunehmenden massiven Luftnot und Pollakisurie, teilweise verbunden mit Brennen, in der Notaufnahme einer kardiologischen Klinik vor. Bei einem im Labor festgestellten akuten Nierenversagen wurde der Patient mit initialer Verdachtsdiagnose eines kardiorenalen Syndroms in unsere Klinik weitergeleitet. Laut Patient sei der Urin trüb gewesen. Darüber hinaus sei eine seit etwa 2 Wochen zunehmende Schwellung der Beine mit Beinumfangdifferenz (re. > li.) aufgefallen. Ambulant wurde bereits mittels farbkodierter Dopplersonographie (FKDS) eine akute Phlebothrombose und Thrombophlebitis ausgeschlossen. Ein chronischer Nikotinabusus wird verneint.
