Diagnosis of Guillain–Barre’ syndrome (GBS) relies on clinical presentation, immunological criteria, electrophysiology, neuroimaging studies if available [16]. Kimachi [4] reported a paraparetic GBS with reversible conduction failure in lower limbs, showing on MRI enhancement of ventral cauda equina (CE) roots. Berciano [1, 2, 5] concluded that enhancement of intrathecal ventral roots is constant finding in early acute motor GBS, both in axonal and demyelinating subtypes. We experienced three novel cases of post-infectious paraparetic GBS with reversible conduction abnormalities restricted to the lower limbs, sequentially studied clinically, electrophysiologically and neuroradiologically over several months. We want to make some brief comments and to acknowledge the supportive role of the imaging, especially in atypical cases. …
