Early-onset-Pneumonien nach prähospitaler Reanimation

Inzidenz nach milder intravasaler Hypothermiebehandlung

Zusammenfassung

Hintergrund

Das gezielte Temperaturmanagement (TTM) ist eine effektive Therapiemethode zur Neuroprotektion bei Patienten nach überlebtem prähospitalem Herzstillstand (OHCA). Erste Veröffentlichungen über diese Therapiemethode unter Verwendung externer Kühlverfahren zeigten eine höhere Rate von Infektionen bei Patienten unter TTM. Ob invasive Hypothermieverfahren ebenfalls mit einer erhöhten Zahl an Infektionen einhergehen, wurde bisher nicht untersucht.

Methoden

Retrospektiv wurde die Inzidenz von früh auftretenden Pneumonien („early onset pneumonia“, EOP) 72 h nach OHCA bei Patienten analysiert, die mit oder ohne gezieltem Temperaturmanagement (Zieltemperatur: 33 °C) therapiert wurden und mindestens 24 h nach Aufnahme überlebten.

Ergebnisse

Insgesamt wurden 68 OHCA-Überlebende (65 ± 15 Jahre) in die Analyse eingeschlossen. Die häufigsten Ursachen für den erlittenen OHCA waren Myokardinfarkt (35 %), primäres Kammerflimmern (24 %), Asystolie (15 %) und Lungenembolie (7 %). Von diesen Patienten wurden 48 % (n = 32) mit TTM behandelt. Die Inzidenz von EOP betrug 42 % unter TTM und 34 % bei Patienten ohne TTM (p = 0,57). Das Auftreten einer EOP war nicht mit einer erhöhten Mortalität assoziiert (Hazard Ratio = 1,02; 95 %-Konfidenzintervall: 0,25–4,16; p = 0,97).

Zusammenfassung

Das gezielte Temperaturmanagement nach OHCA mit einer Zieltemperatur von 33 °C ist eine sichere Behandlungsmethode und war im beschriebenen Kollektiv nicht mit einer erhöhten Infektionsrate assoziiert.
