The clinical diagnosis in patients with parkinsonism has been mainly dependent on neurological manifestations and many of the cases might take a longer time for the final diagnosis [
1]. However, the early differentiation of atypical parkinsonian disorders from Parkinson’s disease (PD) is important not only to manage patients with parkinsonism but also to predict the prognosis or perform clinical trials. Herein, we report the case of a patient presenting with a sustained response to levodopa treatment for akinetic-rigid parkinsonism and satisfaction concerning the diagnostic criteria of progressive supranuclear palsy (PSP) after one-year follow-up [
2]. Interestingly, based on the initial findings of dopaminergic transporter (DAT) imaging, the patient was notably presumed to have PSP [
