Effektivität der glukosesenkenden Therapie bei Patienten mit Typ-2-Diabetes in primärärztlichen Praxen

Zusammenfassung

Hintergrund

Bei Menschen mit Typ-2-Diabetes ist ein optimaler HbA1c-Wert (HbA1c: Glykohämoglobin Typ A1c) zur Risikoreduktion diabetesbedingter Folgeerkrankungen wichtig. Evidenz zur Effektivität glukosespiegelsenkender Wirkstoffe und deren Kombinationen in der Anwendung, zu wichtigen Prädiktoren für die Notwendigkeit einer Zweitlinientherapie und das Nichtansprechen auf eine basalinsulinunterstützte orale Therapie wurde bisher nicht ausreichend zusammengefasst.

Ziel der Arbeit

Es soll eine Übersicht neuer Studien zur Effektivität glukosespiegelsenkender Therapien bei Menschen mit Typ-2-Diabetes im Setting primärärztlicher Praxen mit dem Fokus auf Deutschland gegeben werden.

Material und Methoden

Relevante aktuelle Studien auf Basis von Routinedaten oder Datenbanken wurden in MEDLINE gesucht, thematisch zusammengefasst und diskutiert.

Ergebnisse

In den letzten Jahren verbesserte sich die glykämische Einstellung bei Personen mit Typ-2-Diabetes in Deutschland. Dennoch erreichen unabhängig von der glukosespiegelsenken Therapie 40 % von ihnen keinen optimalen HbA1c-Wert von <7 % [53 mmol/mol]. Der HbA1c-Wert zu Beginn der Behandlung stellt einen wichtigen Einflussfaktor für die weitere Entwicklung der glykämischen Einstellung dar. Darüber hinaus sind eine frühe Intensivierung der glukosespiegelsenkenden Therapie, jüngeres Alter bei der Diabetesdiagnose und ein niedriger Body-Mass-Index wichtige Faktoren zur Reduktion und späteren Stabilisierung des HbA1c-Werts.

Diskussion

Die Effektivität glukosespiegelsenkender Therapien bei Typ-2-Diabetes in Deutschland kann weiter gesteigert werden. Des Weiteren erhalten Patienten bisweilen zu spät eine geeignete Kombinationstherapie, die den HbA1c-Wert besser reduziert als Monotherapien.
