Einfluss der Atemluftbefeuchtung auf die Akzeptanz der CPAP-Therapie bei Patienten mit obstruktiver Schlafapnoe

Zusammenfassung

Hintergrund

Die Compliance gegenüber der kontinuierlichen Überdrucktherapie („continuous positive airway pressure“, CPAP) ist u. a. durch Nebenwirkungen an den oberen Atemwegen limitiert. Befeuchter können diese reduzieren, jedoch ist die Wertigkeit einer routinemäßigen Verordnung unklar. Ziel der Studie ist ein Vergleich von CPAP mit und ohne Befeuchtung hinsichtlich Nebenwirkungen, Akzeptanz und Compliance.

Methoden

Insgesamt 74 Patienten mit OSA wurden in der Studie randomisiert und einer CPAP-Therapie ohne (OB) bzw. mit Befeuchter (MB) zugeordnet. Die Symptome wurden vor Therapie sowie nach 3 Monaten mittels Fragebogen erfasst und die Akzeptanz sowie subjektive und objektive (Betriebsstunden) Compliance dokumentiert.

Ergebnisse

Von den Patienten gaben 67% (OB) und 91% (MB) an, das Gerät jede Nacht zu nutzen. Trotz eines Trends zugunsten der Therapie MB unterschieden sich die Betriebsstunden und die Akzeptanzraten nicht statistisch signifikant voneinander. Die nasalen Symptome zeigten in beiden Gruppen eine vergleichbare Verbesserung.

Schlussfolgerung

Die Akzeptanz und die objektive Compliance gegenüber CPAP scheinen unabhängig von der routinemäßigen Verordnung eines Befeuchters und die subjektiven Symptome unter Therapie unterscheiden sich nicht. Die grundsätzliche Verordnung eines Befeuchters erscheint nicht gerechtfertigt.
