Einsatz der physikalischen Therapie bei der Behandlung der kraniomandibulären Dysfunktion

Eine prospektive, randomisierte klinische Studie

Zusammenfassung

Hintergrund

Ob der zusätzliche Einsatz einer kombinierten physikalischen Therapie, wie sie bei kraniomandibulären Dysfunktionen (CMD) häufig Anwendung findet, einen additiven positiven Effekt auf das Therapieergebnis hat, wird kontrovers diskutiert. In einer prospektiven randomisierten Studie sollte dies anhand objektiver und subjektiver Parameter untersucht werden.

Patienten und Methoden

Insgesamt 26 Patienten mit arthrogener CMD und schmerzhaft eingeschränkter Mundöffnung wurden in zwei Gruppen eingeteilt. Die eine Gruppe erhielt ausschließlich eine Michigan-Schiene, die andere zusätzlich physikalische Therapie. Vor Behandlungsbeginn erfolgte eine klinische Untersuchung, wobei die Unterkieferbewegungen elektronisch registriert wurden. Der Grad des subjektiven Schmerzempfindens wurde anhand einer visuellen Analogskala erfasst. Am Ende der dreimonatigen Behandlung wurde die Verbesserung der Beschwerden ermittelt.

Ergebnisse

Im Vergleich zu den Ausgangswerten verbesserte sich nach Behandlung in beiden Gruppen das Ausmaß der Unterkiefermobilität signifikant, während der subjektiv empfundene Schmerz signifikant abnahm (p<0,05).

Schlussfolgerung

Die physikalische Therapie scheint eine zusätzliche positive Wirkung auf das Behandlungsergebnis bei Patienten mit CMD zu haben.
