Zusammenfassung

Diese Übersicht zeigt aktuelle Trends und Entwicklungen in der elektrischen Device-Therapie und Risikostratifikation auf. Unverändert stellt der plötzliche Herztod auf dem Boden von ventrikulären Herzrhythmusstörungen eine der häufigsten Todesursachen in Industrienationen dar. Die Mehrzahl der Defibrillatoren in Deutschland wird seit Jahren aus primärprophylaktischer Indikation implantiert. Neuere Studien zeigen aber, dass insbesondere bei nichtischämischer Kardiomyopathie oder eingeschränkter Nierenfunktion der Nutzen der ICD(implantierbarer Kardioverter-Defibrillator)-Therapie zumindest bei einem Teil der Patienten auch angesichts einer über die Jahre verbesserten, medikamentösen wie Resynchronisationstherapie überschätzt wurde. Das Risiko eines plötzlichen Herztodes hat insbesondere bei Patienten mit Herzinsuffizienz und verminderter linksventrikulärer Ejektionsfraktion und/oder CRT(kardiale Resynchronisationstherapie)-Versorgung in den vergangenen Jahren abgenommen. Trotz kontinuierlicher Weiterentwicklung sind Elektrodendysfunktionen, Infektionen der transvenösen ICD-Systeme sowie Einschränkungen bei Zugangsproblemen eine zunehmende Herausforderung. Das rein subkutane ICD-System stellt vor diesem Hintergrund eine attraktive Option dar, für die allerdings randomisierte Daten zu Effektivität und Sicherheit bislang fehlen. Relativ simple neue Risikoscores wie der PADIT-Score ermöglichen ein Abschätzen des Infektionsrisikos bei Device-Operationen, und Ergebnisse zu neuen Antibiotika-beschichteten Schutzhüllen liefern vielversprechende neue Therapieoptionen für Device-Operationen. Im Bereich der Schrittmachertherapie gewinnt die Resynchronisation mittels His-Bündel-Stimulation anhand aktueller Studien an Bedeutung, wenngleich wie für den S‑ICD (subkutan implantierbarer Kardioverter-Defibrillator) randomisierte Daten abgewartet werden müssen, um die zukünftige Rolle der Therapie beurteilen zu können.

SGLT-2-Inhibitoren für alle Patient:innen mit chronischer Herzinsuffizienz empfohlen

Das ESC-Leitlinien-Update 2023 bedeutet einen Paradigmenwechsel in der Behandlung der Herzinsuffizienz (HF), denn nun werden SGLT-2i sowohl für HFrEF, als auch für HFmrEF und HFpEF empfohlen. Dapagliflozin als neue Therapieoption zur Behandlung einer Herzinsuffizienz unabhängig von der Ejektionsfraktion

Dapagliflozin ist nun zur Behandlung aller Patient:innen mit chronischer symptomatischer Herzinsuffizienz zugelassen und bietet somit auch neue Hoffnung für die Therapie von jenen mit HFpEF. In der DELIVER-Studie zeigte der SGLT-2-Inhibitor eine signifikante Reduktion von Herzinsuffizienz-Hospitalisierungen und CV-Todesfällen.

ACS-Erstlinientherapie: Konsensbeschluss rät zur DAPT mit Ticagrelor

Ein Konsortium führender Fachgesellschaften erarbeitete jüngst auf Basis umfangreicher Metaanalysen einen Konsens für die Therapie koronarer Herzkrankheiten. Was dabei auffiel: Die duale Plättchenhemmung (DAPT) mit Ticagrelor ist die bevorzugte Therapieoption für das akute Koronarsyndrom (ACS).
