Zusammenfassung

Die infektiöse Endokarditis (IE) ist eine meist bakteriell verursachte hochkomplexe Multisystemerkrankung kardiovaskulärer Strukturen, die trotz technologischer Fortschritte in der Diagnosestellung und Therapie unverändert mit einer Mortalität bis zu 20 % assoziiert ist. Die Diagnostik und die Therapie erfordern den Einsatz eines interdisziplinären Endokarditis-Teams. Die antimikrobielle Therapie stellt eine anerkannte Behandlungsmethode der IE dar, obwohl bei bis zu 50 % der behandelten Patienten im Verlauf der Behandlung eine chirurgische Sanierung erfolgen muss. Die 3 Hauptindikationen für eine frühzeitige Operation sind das progrediente kardiale Versagen, die unkontrollierte Infektion bis hin zum septischen Schock und die Prävention systemischer Embolisationen. Ziel des vorliegenden Beitrags ist es, einen Überblick über den Stand der aktuellen Leitlinien zu Diagnostik und Therapie der IE zu geben.
