Die Empfindlichkeit des nozizeptiven Systems ist nach einer Gewebeschädigung durch periphere und zentrale Sensibilisierung gesteigert. Hieraus resultieren Hyperalgesie und Allodynie, normalerweise eine relativ kurz dauernde Modulation des nozizeptiven Systems, die in eine lang dauernde Modifikation mit veränderter Genexpression übergehen kann. Die molekularen Mechanismen der so entstehenden Schmerzchronifizierung sind klinisch bisher nicht messbar. In der praktischen Schmerztherapie sollte man als Arbeitshypothese davon ausgehen, dass die Chronifizierung ein reversibler Prozess ist. Behandlungsansätze können sich ex iuvantibus aus der diagnostischen Lokalanästhesie und aus pharmakologischen oder verhaltensmedizinischen Eingriffen in Gedächtnisprozesse ergeben. Das implizite assoziative Gedächtnis, das in Form von klassischer oder operanter Konditionierung an der Chronifizierung von Schmerz beteiligt ist, wird umgekehrt auch zur Behandlung chronischer Schmerzen eingesetzt. Die Verhinderung der Konsolidierung des Schmerzgedächtnisses sowie seine Extinktion sind zentrale Ziele einer rationalen Schmerztherapie.
