Zusammenfassung

Entzündliche Lungenerkrankungen in der Schwangerschaft sind relativ seltene Ereignisse. Diese Tatsache spiegelt sich in der spärlichen Literatur zu diesem Thema wider. Die wesentlichen Informationen stammen in der Regel aus kleinen, meist retrospektiven Kohortenstudien. Die verfügbaren Informationen zeigen keinen wesentlichen Unterschied im Verlauf und dem Vorkommen solcher Erkrankungen bei schwangeren und nichtschwangeren Frauen. Letztlich gibt es auch keinen Beweis, dass die bekannten physiologischen Veränderungen der Immunreaktion bei Schwangeren tatsächlich auch zu einer klinisch signifikanten Immunsuppression führen, die mit einer Steigerung der Infektionsanfälligkeit im Respirationstrakt verbunden ist. Trotzdem stellen in der Schwangerschaft auftretende Pneumonien eine erhebliche Gefährdung für die werdende Mutter dar. Bekanntermaßen sind Schwangere mit einer präexistenten Immunsuppression (z. B. durch HIV-Infektion) häufiger betroffen. Die Diagnostik und Therapie von Pneumonien in der Schwangerschaft ist zwar häufig erschwert, entspricht aber im Wesentlichen der bei nichtschwangeren Erwachsenen und muss lediglich bei zu befürchtender fetaler Schädigung modifiziert werden.
