Fahreignung bei autoimmunen Anfällen und autoimmun-assoziierten Epilepsien

Zusammenfassung

Die aufgehobene Fahreignung ist eine der einschneidendsten Folgen für Menschen mit neu aufgetretenen epileptischen Anfällen. Rezidivierende Anfälle im Rahmen einer Autoimmunenzephalitis (AE) treten oft mit Antikörpern (AK) gegen Oberflächenantigene auf. Dies sind in der Regel der N‑Methyl-D-Aspartat-Rezeptor (NMDAR), Leucine-rich-Glioma-Inactivated-Protein 1 (LGI1) oder Contactin-assoziiertes Protein‑2 (CASPR2). Es handelt sich um akut-symptomatische Anfälle, die häufig und anhaltend unter adäquater Therapie sistieren. Allerdings finden sich in den aktuellen deutschen Fahreignungsleitlinien keine Vorgaben für sich wiederholende, sondern nur für singuläre akut-symptomatische Anfälle. Angesichts der günstigen Prognose scheint bei solchen Patienten eine Wiederbewilligung der Fahreignung für Gruppe-1-Führerscheine bei guter Überwachung des Krankheitsverlaufes nach 6 Monaten vertretbar. Für Aussagen zur Fahreignung für Gruppe-2-Führerscheine ist die Datenlage noch nicht ausreichend. Für autoimmun-assoziierte Epilepsien (z. B. mit AK gegen Glutamatdekarboxylase [GAD]) gelten die üblichen Vorgaben für Epilepsien anderer Ätiologien.
