Klassifikation myeloproliferativer Neoplasien und chronischer myeloproliferativer Erkrankungen

Zusammenfassung

Die myeloproliferativen Neoplasien (MPN, chronische myeloproliferative Erkrankungen nach alter Nomenklatur) umfassen die chronische myeloische Leukämie (BCR-ABL-positiv, CML), die Polycythaemia vera (PV), die primäre Myelofibrose (PMF), die essenzielle Thrombozythämie (ET), die chronische Eosinophilenleukämie (CEL) und die chronische Neutrophilenleukämie (CNL). Gemeinsam ist ihnen der zweiphasige Verlauf mit einer Überproduktion einer oder mehrerer Blutzellreihen gefolgt von einer hämatopoetischen Insuffizienz aufgrund von Blastenkrise und/oder Markfibrose.
Klassifikation und Prognostik erfordern ein kombiniertes Vorgehen aus morphologischer Untersuchung, sehr häufig mit einer Knochenmarkhistologie und molekularpathologischer Analyse mit Nachweis der charakteristischen Treibermutationen sowie den Verlauf modulierender zusätzlicher genetischer Aberrationen.
