18. Ethische Fragen bei neurodegenerativen Erkrankungen

Zusammenfassung

Die Behandlung von Patienten mit neurodegenerativen Erkrankungen wirft etliche ethische Fragen auf, die am Beispiel der amyotrophen Lateralsklerose diskutiert werden. Bereits aus Verzögerungen der korrekten Diagnose und unnötigen Behandlungsversuchen im Vorfeld ergeben sich ethische Bedenken. Ein früher praktiziertes therapeutisches Privileg des Arztes, die Diagnose dem Betroffenen vorzuenthalten, lässt sich ethisch nicht rechtfertigen. Die Versorgung der Patienten sollte in multiprofessionellen Zentren nach dem neuesten Stand der Wissenschaft erfolgen, doch ergibt sich hier das Problem der Zugangsgerechtigkeit. Offen ist auch die Frage der Berücksichtigung von Angehörigeninteressen bei Therapieentscheidungen. Die Versorgung sollte eine vorausschauende Behandlungsplanung (Advance Care Planning) und Information über Therapiebegrenzung enthalten. Wünsche der Patienten nach Lebensverkürzung sind ernst zu nehmen und sollten in einen tabufreien Dialog und verantwortungsvolles Handeln münden.
