Das alternde Gesicht – Eine anatomische Übersicht

Zusammenfassung

Hintergrund

Alterungserscheinungen im Gesicht entstehen im Laufe des Lebens aufgrund einer Vielzahl altersphysiologischer Veränderungen unterschiedlicher Gewebe wie Haut, Knochen, Bänder, Fettgewebe und Muskulatur.

Zielsetzung

Es wird eine anatomische Übersicht über die altersphysiologischen Veränderungen der unterschiedlichen Gesichtsgewebe gegeben.

Material und Methoden

In dieser Übersichtsarbeit werden aktuelle Kenntnisse aus klinischen, bildgebenden und anatomisch-deskriptiven Studien zusammengefasst. Dabei wird ein Fokus auf die Gewebe „unterhalb der Haut“ gelegt: Knochen, Bänder, Fettgewebe und Muskulatur.

Ergebnisse

Die physiologischen Veränderungen sind lokal stark unterschiedlich. So fand im Mittelgesicht ein vermehrter Knochenabbau statt, während beispielsweise im unteren Drittel der Stirn die Dicke des Knochens zunahm. Bänder sind nach aktuellen Forschungsergebnissen wahrscheinlich nicht von Alterserscheinungen betroffen. Bänder weisen lediglich aufgrund der altersbedingten Umbauprozesse ihres knöchernen Fundaments Veränderungen in ihrem Verlauf auf. Auch das Fettgewebe verhält sich unterschiedlich: Hierbei sind das oberflächliche Fettgewebe (Schicht II) und das tiefe Fettgewebe (Schicht IV) in unterschiedlicher Ausprägung von Altersprozessen betroffen. Die mimische Muskulatur zeigt im Alter eine verminderte Kontraktilität.

Schlussfolgerung

Die altersphysiologischen Veränderungen im Gesicht sind vielfältig und bei jedem Menschen individuell ausgeprägt. Sichere und effektive Verjüngungstherapien können nur bei einem guten Verständnis der lokalen Veränderungen in den jeweiligen Gesichtsregionen sowie ihrer Wechselwirkungen aufeinander gewährleistet werden.

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
