Folsäure gegen Hyperhomocysteinämie

Ein neuer Ansatz zur Prävention und Therapie Alkoholismus-bedingter Störungen?

Zusammenfassung

Bei Patienten mit Alkoholismus führt der Alkoholentzug zu einer Erhöhung exzitatorischer Aminosäuren (Glutamat, Aspartat, Homocystein). Eine Alkoholismus-assoziierte Hyperhomocysteinämie kommt nicht nur im Alkoholentzug vor, sondern ist mit kontinuierlichem Alkoholkonsum verknüpft. Der massive Anstieg der Plasmahomocysteinkonzentration bei Alkoholismus mit aktivem Trinkverhalten zeigt ein plausibles pathobiochemisches Korrelat zu vaskulären Komplikationen während des Alkoholentzugs wie z.B. zu akuten zerebralen Ischämien oder Entzugsanfällen. Vor dem Hintergrund einer unter Alkoholentzug verstärkten glutamatergen Exzitotoxizität und Stress-induzierter Permeabilisierung der Blut-Hirn-Schranke erlauben erhöhte Homocystein-Plasmakonzentrationen die Prädiktion von Alkoholentzugsanfällen, was dann die frühzeitige Einleitung einer medikamentösen antiepileptischen Prophylaxe ermöglicht. Des weiteren ist kürzlich gezeigt worden, dass eine Hyperhomocysteinämie mit einem erhöhten Risiko dementieller Erkrankungen verschiedener Art korreliert ist. Auch die Alkoholismus-induzierte Hyperhomocysteinämie korreliert mit der hippocampalen Volumenreduktion bei Patienten mit Alkoholismus und könnte somit zur Pathogenese auch der Alkohol-bedingten Hirnatrophie beitragen. Die Therapie mit Folsäure (plus Vitamin B6 und B12) zur Senkung erhöhter Plasma-Homocysteinkonzentrationen könnte daher nicht nur die etablierte Therapie der Alkoholentzugssymptomatik unterstützen, sondern auch die Entwicklung einer alkoholbedingten Hirnatrophie verlangsamen. Prospektive Interventionsstudien werden klären, ob durch diese flankierende Therapie über eine frühzeitige Senkung erhöhter Homocysteinspiegel die Inzidenz von Komplikationen des Alkoholentzuges oder Alkoholismus-induzierter chronischer Schäden reduziert werden kann.
