Gastrointestinale Nebenwirkungen und Ernährung bei Tumortherapie

Zusammenfassung

Bereits bei Diagnosestellung ist eine Mangelernährung bei vielen Tumorpatienten evident. Insbesondere in der palliativen Situation werden Tumorpatienten über große Spannen der ihnen verbleibenden Zeit mit systemischen Tumortherapien mit dem Ziel behandelt, die Lebensqualität zu erhalten oder zu verbessern. Unerwünschte Wirkungen am Intestinaltrakt können zu einer weiteren Verschlechterung des Ernährungszustands beitragen und so neben der Beeinträchtigung der Lebensqualität den Nutzen der Therapie begrenzen. Mundschleimhautentzündungen, Übelkeit und Erbrechen, aber auch Diarrhö oder Obstipation führen häufig zu einer Einschränkung der Nahrungsaufnahme. Die Möglichkeiten der Vorbeugung und Behandlung der gastrointestinalen Nebenwirkungen der Tumortherapie wurden in den letzen Jahren erheblich erweitert. Es liegen von verschiedenen Fachgesellschaften Leitlinien und Empfehlungen vor, die bei der Wahl geeigneter Maßnahmen eine rasche Orientierungshilfe darstellen. Können trotz optimalen Einsatzes der verfügbaren Maßnahmen die Nebenwirkungen nicht abgestellt werden, muss der Nutzen der Therapie kritisch gegenüber den Belastungen abgewogen werden.
